US doctor to Germany: recognition and licence roadmap

A US medical degree does not itself permit clinical practice in Germany. First identify the state licensing authority and Approbation requirements, then coordinate language, equivalence, specialist recognition, employment and immigration as linked but separate files.

Approbation is the clinical-practice gate

Doctors are regulated professionals. The federal recognition portal states that Approbation is mandatory to work as a doctor in a hospital or open a practice. A job offer, US state licence or immigration approval does not replace German professional authorisation.

Responsibility sits with the competent authority in the federal state where you intend to work. Choose a realistic state and use the official Recognition Finder result to contact that authority. Do not prepare a generic national packet before checking its current state-specific list.

Build an authority-ready education and standing file

Start with identity, medical diploma, detailed education and training records, professional experience, current or previous professional authorisations and evidence of good standing as requested by the authority. Records from a university, residency programme, licensing board and employers can take different amounts of time, so request them against the authority checklist rather than from memory.

Ask whether documents must be sent directly, certified, apostilled or translated, and which translators are accepted. The general recognition guidance says not to send originals and notes that authorities may ask for certified copies or German translations. Preserve the original-language document, certification, translation and dispatch proof as one set.

Plan equivalence and language as decision points

The authority evaluates the foreign professional qualification and tells you what further evidence or measures are needed. Do not assume a particular examination outcome before the individual assessment. Ask how professional experience is considered and what review or remedy is available if substantial differences are identified.

Obtain the authority’s current requirements for general German and medical language, including the accepted certificate, level, test provider and validity. Separate everyday language preparation from clinical communication practice; both can affect the path to lawful, safe work even when the immigration route has its own language rules.

Treat specialist recognition as a later, separate decision

If you completed residency or fellowship training in the United States, do not describe yourself as a German-recognised specialist until the competent medical chamber decides the case. The official doctor page says recognition of specialist qualifications is possible only after Approbation has been granted.

Ask the relevant chamber for its curriculum, rotation, procedure-log and employment-certificate requirements. Sequence matters: prepare records early, but keep Approbation and specialist recognition as separate milestones in the plan and in employer discussions.

Coordinate the employer and immigration file

Tell prospective employers exactly which recognition stage is complete, which decision is pending and what duties you may lawfully perform. Make the clinical start and job title conditional on professional permission. At the same time, test the offer against the relevant German work route; professional licensing and immigration are both necessary where each applies.

Before travel, confirm whether your nationality requires a national visa and whether any recognition-focused route is more appropriate than immediate qualified employment. Use the German mission and immigration authority for the actual application; the recognition authority cannot grant residence permission.
